Support Coordination
A capacity building support that helps you put your NDIS plan into action.
What it is
Your coordinator works for you
Support coordination helps you understand your funding, connect with the right providers, and build the confidence to manage your supports over time. Your coordinator works for you, not for any one service provider.
Level 2
Coordination of Supports (Level 2)
This is ongoing, practical help to understand and use your NDIS plan. Your coordinator helps you connect with and manage a mix of funded, mainstream, community and informal supports, resolve any issues that come up with your services, and build your confidence to direct your own supports over time.
This may suit you if
- You have a new plan and are not sure where to start.
- You have several providers to coordinate.
- You want guidance without losing control of your decisions.
Level 3
Specialist Support Coordination (Level 3)
This is a higher level of support coordination for people facing more complex or higher risk circumstances, such as housing instability or several service systems to manage at once. It focuses on reducing risk, planning for and responding to crisis situations, and coordinating across health, housing, justice and disability systems. This support is usually time limited and delivered by a suitably qualified and experienced coordinator.
This may suit you if
- Your situation involves complex or high risk circumstances.
- You are dealing with housing instability.
- You need to coordinate across multiple service systems at once.
Inside The Work
Open each level to see how we actually deliver it.
No jargon. Here is what actually happens.
Coordination of Supports (Level 2)
- Your plan reviewed line by line so every dollar is accounted for
- A shortlist of providers matched to your goals, not just what happens to be available
- Service agreements checked before you sign anything
- Regular check ins to catch problems early, not after they escalate
- A single point of contact for every provider you work with
- Paperwork and preparation ready well before your plan review
Specialist Support Coordination (Level 3)
- Risk and complexity assessed from day one
- A coordinated plan across housing, health and disability systems
- Direct liaison with hospitals, housing services and clinical teams where relevant
- A crisis response pathway agreed before it is needed, not after
- Reporting shared openly with your broader care team
- Reviews focused on stabilising your situation, then building your independence
